Which Form Is Required for Address Transfer or Correction?
For both:
- Voter ID address transfer
- Correction in existing voter details
You must fill Form 8.
Click on “Fill Form 8” to proceed.
Choose Application Type
You will see two options:
- Self
- Other Elector
Select Other Elector, then:
- Enter your EPIC number (Voter ID number)

Important Rule for Married Women
Married women cannot change everything at once.
Step 1: Address Shift
- First, apply for Shifting of Residence
- Keep maiden surname
- Keep father’s name as relative name
Step 2: Correction
After the shift is approved:
- Fill Form 8 again
- Change:
- Surname
- Relative name (husband’s name)
